Is an integral component of a
return to work and
rehabilitation programme.
We dedicate an Occupational
Health Advisor as case manager
to individual companies. The
case manager liaises with the
employee, Human Resources, the
appropriate rehabilitation
specialists and the General
Practitioner.

Monitor the employee’s
progress towards recovery.

Partner Health will take
responsibility for the
administrative tasks
associated with your
employee’s treatment in
referrals, review of the
reports, initiating
recommendations and keeping
relevant parties informed of
progress
Medical Case
Management
After the initial sickness
absence assessment of the
employee, Partner Health
assumes responsibility for
the co-ordination of the
employee’s treatment to
accelerate recovery to
complete health and return
to work through
rehabilitation.
This involves evaluation of
the extent, severity and
impact of the illness
Review the current treatment
plan and co-ordinate
referrals to diagnostic
services and treating
specialists
Liaise with the employee GP
Review the employee progress
towards recovery
Graded return to work
program for the employee
Ensuring that they are
getting the most appropriate
and effective treatment
Facilitate the referral to
the appropriate Medical
Specialists,
Physiotherapists,
Psychologists and
counsellors
